Here is an opportunity to join our company as an Admissions Coordinator. As an Admissions Coordinator, you will be an integral member of the Operations team with responsibility for coordinating all Home Care admissions to the agency. Additionally, you will assist and support Medicaid Waiver candidates through the Maximus process with a goal of successful attainment of the Community Health Choices waiver benefit. The ideal candidate has a healthcare related college degree and prior healthcare sales, marketing, or Intake Department experience.

Primary Responsibilities:

  • Personal vehicle required as candidate will be performing routine home visits to consumer's place of residence.
  • Responsible to respond to referral inquiries promptly and complete the full cycle intake process for skilled services, non-skilled services and private pay.
  • Achieve a turnaround time of 15 minutes for all private pay and skilled services referrals, and a turnaround time of 24 hours for all non-skilled service referrals.
  • Receives incoming calls and makes outbound calls to follow up lead referrals, state agencies, physicians, and partners, etc.
  • Responsible to promote all services offered through the company to potential participants.
  • Has an in-depth knowledge of Blessings4Ever verticals, services, benefits, and open positions.
  • Responsible to understand and comply to various state waiver programs and private pay initiatives.
  • Develops professional relationships with outside agencies to better assist incoming participants.
  • Adhere to policies, procedures, and compliance requirements of Blessings4Ever Home Care Agency for OLTL.
  • Work in conjunction with Marketing to scout for new business opportunities and client leads.
  • Work in conjunction with Marketing/Clinical Liaison to effectively funnel all lead referrals through the intake department pipeline.
  • Work in conjunction with Marketing to establish customer experience events and practices to maximize positive employee outcomes, promoting the mission vision and values of B4E, and improving customer experience with Blessings4Ever Homecare Agency.
  • Identify opportunities to be proactive in consumer cases as well as intervening on their behalf before the issue actually occurs in conjunction with the CEM and clinical liaison.
  • Communicate customer service feedback to the CEM and/or Clinical Liaison.
  • Maintaining a positive and empathetic and professional attitude towards employees and consumers at all times.
  • Monitors gaps and absence of activity on HHA report coming from Finance to ensure that services are rendered. Work in collaboration with the PCA recruiter, Eligibility coordinator and Case Management department to ensure the proper follow up was completed before requesting discharge.
  • Ensure that all incoming PCAs are in compliance and that documentation is acquired.
  • Other duties as assigned.

Will Demonstrate the following Skills:

  • Must be comfortable on the phone with potential consumers and caregivers promoting company offers.
  • Ability to present to management analysis of current trends, required solutions,
  • Must have proficient skill in sales.
  • Must be comfortable contacting state agency representatives.
  • Minimum 2 years of experience in customer service, sales, or a healthcare related field.
  • Proven effectiveness to interact with a high volume of consumers.
  • Ability to keep sequential and detailed documentation of referrals related to referral management and conversion process and further ability to track, trend and analyze ADC, Lost referrals, trends with conversion ratios.
  • Strong computer skills.
  • Ability to maintain a cooperative and respectful relationship with co-workers and others by communicating information; responding to requests; building rapport; participating in team problem-solving; conducting self in a professional manner;
  • Possess solid verbal and written communication skills.
  • Must be detail-oriented.
  • Work independently, demonstrate organizational and problem-solving skills.
  • Strong knowledgebase of our products and service lines.
  • Must possess ability to multi-task and problem solve.
  • Bilingual Spanish preferred, but not required.

Education/Experience Requirements

  • Bachelors degree preferred.
  • High school diploma or equivalent required.
  • At least 1 -2 years of experience in a health care or home care environment preferred.
  • Bilingual: Spanish/English.
